Where is your office?
My office is located at 5400 Ward Rd in Arvada, CO. The office is in the brown building complex, Building V, on the east side of the lot. There is ample parking available. Once you enter the main door, I am the corner office to the left, Suite 110. Please enter and make yourself comfortable in the waiting room.
My office is located at 5378 Sterling Dr, Studio 5, Boulder, CO 80304. From Valmont St, you will see a sign for 5480 The Valmont Building on your right. Make the first left after this sign. The office is in a free-standing brick building. You will be at the back of the building. You are free to use any of the parking spots in the lot. You will see the entrance door at the back of the building. Please enter and make yourself comfortable in the waiting room.
How do I make an appointment?
Please provide your contact information here. Or, you can call the direct line at (720) 727-7188. Voicemail and email are confidential. You will be contacted within 24 hours on business days and I will set up a time to either speak to
you by phone or schedule an initial intake session.
What happens after I contact you?
We will schedule a time for a complimentary phone consultation. This is the time to ask questions and be curious! I get to understand a little bit about what you are experiencing to see if I am a good fit as your therapist. I can answer any questions you may have, including about finances and insurance. Clients report feeling more comfortable and relaxed when coming in for their first session after a phone consult.
Once we determine we would like to move forward, our Executive Assistant at C.R.E.A.T.E. Outcomes will contact you with an invitation to our client portal. The email sent will contain the details of our appointment time, directions to the office, and a request for you to submit paperwork through our portal.
When your appointment time comes, we will spend the first few sessions getting to know each other. I will be learning about who you are, what you may be struggling with, and what you have been through in your life so far. Then, we will collaborate to create your treatment plan by creating a life map together. I am happy to explain this process in more detail during your phone consultation!
